The theme of the day

Something moved faster than your notes did. A project, a process, a set of decisions — the reality of where things are has gotten ahead of the documentation, the tracker, the mental model you built to keep it organized. The current state lives in conversations, in your head, in recent decisions that haven’t been formally recorded anywhere, and the gap between that and the official record is starting to create friction.

This is a particular discomfort because the whole point of the system was to not have this problem. The tracker was supposed to be ground truth. The notes were supposed to carry context forward. But things moved quickly, or the system was built for an earlier version of the project, or some combination — and now the map and the territory are no longer matching.

The question isn’t how to prevent this from happening again. It’s what to do with the gap right now. The first move is figuring out which records are worth updating and which ones have simply been outrun and can be retired without ceremony.

In work contexts, outdated documentation creates real costs — duplicated effort, decisions made on stale information, time spent re-explaining things that should have been captured. Notice where the most expensive gaps are: the places where the disconnect between documentation and reality is actively shaping how decisions get made or how work gets done.

Not everything needs to be brought current. Some records served a moment that has passed and don’t need to follow the project into its next phase. Being selective about what to update is itself a form of precision, not a shortcut.

What to notice today

Which specific piece of information, if it were accurately recorded, would remove the most friction from today’s work. Start there rather than trying to bring everything current at once.

Also notice whether the gap between documentation and reality has been creating a secondary problem — a low-level unease about being caught out, or a reluctance to bring others into the current state because the official record doesn’t reflect it.

Documentation serves the work, not the other way around. Update what matters, retire what doesn’t, and release the rest.
